Information is not passed on without your consent unless it is within the confines of the NHS, by legal framework, or is in the public interest. Certain anonymised patient data may be shared for the purposes of public health and audit, research, teaching and training. This practice is registered under the Data Protection Act. It is a practice and legal requirement that all staff maintain confidentiality of patients' records.

  Patients new to the practice are invited for a consultation and medical check and should bring their medical card to register. Patients have the right to choose which doctor they would prefer to register with.

Telephone Advice   Repeat Prescriptions
     
If you would like advice please telephone the surgery number after morning surgery, i.e. after 11.30am. This reduces interruptions during consultations. Please phone the surgery number allowing 24 hours notice. The prescription will be ready for collection after 4pm the following day.
   
Over-75 Health Checks Dispensing of Medication
   
 
Registered patients aged over 75 who have not been seen in the previous 12 months may request a consultation. If you are unable to attend the surgery

for this consultation because of your medical condition, a home visit may be arranged.

 

Travel Information

 

Middle Lane Surgery has a nurse specialising in travel medicine and immunisations and the surgery is a registered Yellow Fever Centre. If you require any vaccinations relating to foreign travel, you can make an appointment with the practice nurse to discuss your arrangements. To assist this process, you will be asked to complete a travel questionnaire. For information on vaccinations recommended for your designated country of travel, please visit www.fitfortravel.nhs.uk.

 

It is important to make the initial appointment as early as possible (at least 8 weeks before you travel) as a second appointment will be required with the practice nurse to actually receive the vaccinations. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

 

Most travel vaccines are ordered on a private prescription and these incur a charge. This is because travel vaccination is not included in the services provided by the NHS and is therefore a chargeable service.

 

We can also advise on suitable drugs, such as anti-malaria tablets, needle packs and other travellers equipment.

  We are able to dispense medication to patients living in rural areas and more than one mile from a pharmacist.
